Now, more than ever, your participation in the TNA Patient Registry by completing a survey is an opportunity to contribute to scientific research and expand what we know about TN and related facial pain. It could yield significant benefits, and influence new treatment options. Please join us in this effort to build hope for the future.
Please take a few moments to complete the "Face Pain Survey" online by using the links provided, or if you would prefer, you may print the form and return it to our national office (click here) at 925 NW 56th Terrace, Suite C, Gainesville, FL 32605-6402.
When you complete the Patient Registry Survey, you will create an individual profile that no one can access but you, using the password of your choice. Your TN and face pain information along, with that of others, can be queried to gather aggregate information in one category or another. Because the TNA Patient Registry information is private and confidential to the patient only, we ask that you also complete a survey for use by the office staff as they work to find the appropriate information and referrals for your specific situation.
